Privacy Practices Notice
This notice is required by federal and state law and explains how we use and disclose your personal health and financial information. Individuals covered by a self-funded employer group health plan will receive this notice from their employer. To request a copy, contact your employer.
California Privacy Rights Act (CPRA) Privacy Notice
This notice is required by California law and explains your rights under CPRA. CPRA applies to California residents only and does not apply to information collected from health plan members or applicants. Members should refer to the Privacy Practices Notice above.
Consent Management Portal
Federal law requires us to provide current members who have Medicare, Medicaid and plans purchased through the Health Care Marketplace with access to certain personal information through applications of their choice. If you would like to consent to make your information available to yourself, you can manage your consent at any time by going to the Consent Management Portal.
Choosing an Application to Access Your Health Information
This notice provides helpful information about who is eligible to access certain types of data through an online application (web app) that you use. If you have questions, or if you are an Authorized Agent, Legal Guardian or Power of Attorney and have questions, please call the number on the back of your insurance card.
